Program
Dear Parents:
As a result of a recent Federal Law, and implementation by New York State, a new vaccination program has been developed:
Affects those families who are either uninsured, or underinsured for immunizations for their children. There is no other restriction to receiving immunizations under this program. Children who would normally receive their vaccinations through a medical insurance benefit are not eligible for this program.
Allows Putnam Valley Pediatrics to offer immunizations for only an administration fee (fifteen dollars) -- the vaccine itself is supplied at no cost by the State and Federal programs.
The vaccines are given in our office at the usual intervals during regular visits, just as we have done in the past.
Children up to the age of 19 years are covered by the law.
All recommended vaccinations are covered under the program.
If you feel that your children are eligible for this program, please speak to the nurse when your child is being prepared for their examination. There is a brochure you must review, and a form that must be signed.
This program has been developed to improve access to immunizations for all our children, and to remove financial barriers preventing appropriate immunization of all children.
